Insurance benefits information obtained through this form should be verified directly with the patient's insurance carrier or benefits administrator before rendering treatment or making coverage determinations.

Authorized Users Only

No Guarantee of Reimbursement

Verification of benefits does not guarantee that an insurance claim will be approved, paid, or reimbursed at the levels indicated in the verification. Final determination of coverage and reimbursement is made solely by the insurance carrier.

Limitation of Damages

In no event shall ChiroEdge be liable for any direct, indirect, incidental, special, consequential, or punitive damages arising from or related to the use of this form or the information obtained through it, including but not limited to loss of revenue, loss of profits, or loss of business opportunity, even if ChiroEdge has been advised of the possibility of such damages.

HIPAA Compliance Notice

If you are not a ChiroEdge client with an executed Business Associate Agreement (BAA), you should not submit Protected Health Information (PHI) through this form. Unauthorized submission of PHI may constitute a violation of HIPAA. ChiroEdge shall not be held responsible for HIPAA violations resulting from improper use of this form.

As-Is Basis

This tool and all information provided through it are made available on an "as-is" basis. ChiroEdge disclaims all warranties, including but not limited to warranties of merchantability, fitness for a particular purpose, and non-infringement.

User Responsibility

Users are solely responsible for the accuracy of any information submitted through this form. ChiroEdge is not responsible for errors, omissions, or inaccuracies in information provided by users, nor for any consequences resulting from such errors.

Indemnification

Users agree to indemnify and hold harmless ChiroEdge, its officers, directors, employees, and agents from any and all claims, damages, losses, liabilities, and expenses (including reasonable attorneys' fees) arising from or related to the user's use of this tool or any information obtained through it.
